Keep Your Eyes Open for Email Request For Participant Registration Forms
As many of you know, Around Town DC is funded by a grant from the DC Department of Aging and Community Living (DACL). DACL requires us to document the number of people served by our program as well as basic demographics information. Funding for Around Town DC programming relies heavily on documenting performance with our service numbers.
We collect this information through a Participant Registration Form; We will be periodically email this form to those who have not yet completed one. We ask that you fill it out as soon as possible once you receive it. All information provided to us is kept confidential. If we do not receive a form from you, you risk not receiving links to join Around Town DC classes.
Thank you for helping us meet DACL's requirement by completing the participant registration form.
